Medicines in school

Please use this form to let us know if you wish your child to be given prescribed medicine whilst in school.  Medicine must be dropped off in the box at the entrance to the school.  This will then be taken to the school office who will check whether a form has been completed.  Ideally we would like the medicine to remain in school however, if this is not possible it will be returned to you in a zip wallet.  At this time we can only accept requests to administer  prescribed medicines.
